The Role of Spasticity in Functional Neurorehabilitation-Part II: Non- pharmacological and Pharmacological Management: A Multidisciplinary Approach
نویسنده
چکیده
The symptom/clinical sign of spasticity constitutes a negative factor when considering FNR for both the quadruped animal and the human biped. The existence of multidisciplinary protocols, based on neuroscience, is essential for patients to develop a coordinated motor function. Thus, a multi-modal approach to spastic movement disorder is required. In the quadruped animal an association between thermotherapy, transcutaneous eletrical stimulation (TENS) (50 Hz for 30 minutes) and resistance training in the under water treadmill (UWTM), followed by passive stretches, for 5 days a week during the initial stages of spasticity, has a significant and positive impact in patients’ motor coordination and functionality. It is possible to assume, through clinical evidence, that, by neuroanatomical comparison, similar protocols will reveal the same level of success if applied to the human biped. An early intervention, associated to a multimodal approach of stretching, thermotherapy, TENS and UWTM allows for a successful FNR. To complete the multidisciplinary approach, in the more severe cases it is possible to provide pharmacological support for the management of neuropathic pain and muscular relaxation. This support can be either local (botulinum toxin) or systemic.

Neuropharmacological Aspects of Cognitive Neurorehabilitation in Epilepsy
The role of CNS neuromodulators in cognitive neurorehabilitation can be related to two main issues: 1) the negative impact on cognition of drug categories prescribed for different neurologic symptoms, such as spasticity, extrapyramidal symptoms, or epileptic seizures; 2) their possible role in neuroprotection and amelioration of the cognitive status of the patient, especially attention and memo...
متن کاملClinical guideline for Non- pharmacological pain management of trauma patients in pre hospital setting
Aims and background: Pain management is the core of care in traumatic injuries, which unfortunately is not properly managed in the pre-hospital stages. The use of pharmacological pain management by emergency medical technicians in the Iran is also subject to widespread restrictions and prohibited, so focusing on nonpharmacological pain management techniques can help improve pain management and...

متن کاملBarrier of implementing non-pharmacological pain management in children and presented intereventions by nurses
Introduction : Researches indicate that children's pain is still not managed well. Pain relief with non pharmaceutical methods in addition to being cost effective it is uninvasive and it only depends on nurses' performance. Objective: This study aims to determine important barriers and presented interventions by nurses for non pharmaceutical pain management in children. &nbs...
متن کاملManagement of spasticity after spinal cord injury: current techniques and future directions.
Spasticity, resulting in involuntary and sustained contractions of muscles, may evolve in patients with stroke, cerebral palsy, multiple sclerosis, brain injury, and spinal cord injury (SCI). The authors critically review the neural mechanisms that may contribute to spasticity after SCI and assess their likely degree of involvement and relative significance to its pathophysiology. Experimental ...
